Behind a cut, a burn or impulsive behavior there is usually <strong>intense emotional pain<\/strong> and difficulties for <strong>regular<\/strong> either <strong>express<\/strong> what it feels like.<br>In this guide we explain <strong>what are they<\/strong>, <strong>how to detect them in time<\/strong> and <strong>what interventions work<\/strong>, with specific guidelines for <strong>educational centers, psychosocial services and mediation teams<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What we mean by self-harm and suicidal behavior<\/h2>\n\n\n\n<p><strong>Self-harm (NSSI)<\/strong>: deliberate damage to one&#039;s own body <strong>no intention of dying<\/strong> (e.g., cuts, burns, bruises). It usually works as <strong>emotional regulation<\/strong>, feeling of control or <strong>self-punishment<\/strong>.<\/p>\n\n\n\n<p><strong>Suicidal behavior<\/strong>: covers <strong>ideation<\/strong>, <strong>plans<\/strong>, <strong>attempts<\/strong> and <strong>completed suicide<\/strong>; exists <strong>intention<\/strong> (or clear risk) of dying.<br>Although they are <strong>different constructs<\/strong>, <strong>often coexist<\/strong> and share risk factors. Evaluate them <strong>jointly<\/strong> improves prevention.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why we are seeing more cases<\/h2>\n\n\n\n<p>In the last decade, multiple surveillance systems and emergency services have observed <strong>more queries<\/strong> for self-harm and ideation\/attempts in adolescents, especially <strong>girls<\/strong>, and in contexts with <strong>bullying<\/strong>, <strong>academic tension<\/strong> either <strong>family crises<\/strong>This has led to <strong>health<\/strong> and <strong>education<\/strong> to prioritize detection and response protocols.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Risk factors (multifactorial model)<\/h2>\n\n\n\n<p><strong>Individuals<\/strong>: depression, eating disorders, impulsivity, low frustration tolerance, previous attempts.<\/p>\n\n\n\n<p><strong>Relatives<\/strong>: chronic conflict, violence, neglect, poor communication, family history of suicide or mental illness.<\/p>\n\n\n\n<p><strong>Social and school<\/strong>: bullying, exclusion, academic pressure, substance use, <strong>access to lethal means<\/strong>.<\/p>\n\n\n\n<p><strong>Cultural and media<\/strong>: <strong>contagion effect<\/strong> due to inadequate coverage, normalization of self-harm on networks.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Warning signs: what to look out for<\/h2>\n\n\n\n<p>Sudden changes in the <strong>mood<\/strong> (sadness, irritability), <strong>isolation<\/strong>, loss of interest.<\/p>\n\n\n\n<p><strong>Wounds<\/strong> or unexplained scarring, persistent use of <strong>long sleeves<\/strong> in heat.<\/p>\n\n\n\n<p>Messages about <strong>empty<\/strong>, <strong>blame<\/strong>, <strong>farewells<\/strong> or \u201cnot make sense.\u201d<\/p>\n\n\n\n<p><strong>Impulsiveness<\/strong>, substance use, <strong>academic decline<\/strong> and <strong>conflicts<\/strong> at home or in the classroom.<\/p>\n\n\n\n<p><strong>Keys to early detection<\/strong><br>To train teachers, counselors, educators and mediators to <strong>ask questions directly and empathetically<\/strong>:<\/p>\n\n\n\n<p>\u201cWhen you feel overwhelmed, have you hurt yourself or thought about hurting yourself?\u201d<br>Questions <strong>clear, without judgment<\/strong> and with <strong>time to listen<\/strong> make it easier for the teenager to open up.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Initial assessment and safety plan<\/h2>\n\n\n\n<p>If you notice any signs, immediately assess:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Current risk<\/strong> (intention, plan, available means).<\/li>\n\n\n\n<li><strong>Frequency and method<\/strong> from self-harm.<\/li>\n\n\n\n<li><strong>Triggers<\/strong> recent (conflicts, losses, bullying).<\/li>\n\n\n\n<li><strong>Support network<\/strong> (family, peers, adult role models).<\/li>\n<\/ol>\n\n\n\n<p><strong>Basic security plan<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Withdraw or <strong>limit access to media<\/strong> dangerous at home\/center.<\/li>\n\n\n\n<li>Identify <strong>early signs<\/strong> and <strong>alternative strategies<\/strong> (breathing, behavioral distraction, contact with reference adult).<\/li>\n\n\n\n<li>List <strong>people and resources<\/strong> to which to turn.<\/li>\n\n\n\n<li>Set <strong>emergency steps<\/strong> (who to notify and how).<\/li>\n<\/ul>\n\n\n\n<p>At risk <strong>high<\/strong>: activate <strong>emergencies<\/strong> either <strong>mental health<\/strong>; at risk <strong>moderate\/low<\/strong>: preferential referral, close monitoring and interprofessional coordination.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Interventions that work<\/h2>\n\n\n\n<p><strong>Cognitive behavioral therapy (CBT)<\/strong> adapted to suicidal\/self-harming behavior: coping skills, problem-solving, cognitive restructuring.<\/p>\n\n\n\n<p><strong>Dialectical Behavior Therapy (DBT) for Adolescents<\/strong>: emotional regulation, tolerance to discomfort, interpersonal effectiveness and <strong>relapse prevention<\/strong>.<\/p>\n\n\n\n<p><strong>Working with the family<\/strong>: improve communication, reduce conflict, <strong>align<\/strong> guidelines and supervision.<\/p>\n\n\n\n<p><strong>School support<\/strong>: anti-bullying protocols, tutoring, access to counseling and safe spaces.<\/p>\n\n\n\n<p><strong>Pharmacotherapy<\/strong>: only for <strong>comorbidities<\/strong> (e.g., depression), <strong>never<\/strong> as the only approach.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The role of mediation with minors<\/h2>\n\n\n\n<p>mediation <strong>does not replace<\/strong> to therapy or clinical intervention, but <strong>Yeah<\/strong> can:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>De-escalate<\/strong> family and school conflicts that maintain the malaise.<\/li>\n\n\n\n<li><strong>Structuring agreements<\/strong> coexistence (use of screens, schedules, study\/leisure times).<\/li>\n\n\n\n<li>Ease <strong>safe dialogue<\/strong> between adolescent, family and educational center.<\/li>\n\n\n\n<li><strong>Coordinate<\/strong> with mental health and social services, respecting <strong>confidentiality<\/strong> and its limits.<\/li>\n<\/ul>\n\n\n\n<p><strong>Good practices in mediation<\/strong><\/p>\n\n\n\n<p><strong>Reviews<\/strong> periodic agreements and <strong>derivation<\/strong> if the risk increases.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Minimum protocol for educational centers and services<\/h2>\n\n\n\n<p><strong>Operational checklist<\/strong><\/p>\n\n\n\n<p><strong>Post-crisis plan<\/strong> (accompaniment, return to classrooms, follow-up).<\/p>\n\n\n\n<p>Procedure of <strong>detection<\/strong> and <strong>derivation<\/strong> clear (who does what and when).<\/p>\n\n\n\n<p><strong>Training<\/strong> annual to faculty and teams (ask, contain, refer).<\/p>\n\n\n\n<p><strong>Security plan<\/strong> type (media withdrawal, key contacts).<\/p>\n\n\n\n<p>Circuit of <strong>coordination<\/strong> with mental health\/social services.<\/p>\n\n\n\n<p><strong>Communication criteria<\/strong> with families and protected registration.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Responsible communication and work with families<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Avoid <strong>minimize<\/strong> (\u201cwants to attract attention\u201d) and also <strong>dramatize<\/strong>.<\/li>\n\n\n\n<li>Replace \u201cwhy are you doing it?\u201d with \u201cwhat were you feeling?\u201d <strong>before<\/strong> to do it?<\/li>\n\n\n\n<li>Agree <strong>clear rules<\/strong> and <strong>predictable<\/strong> at home; reinforce <strong>self-care behaviors<\/strong>.<\/li>\n\n\n\n<li>In networks and internal media, <strong>do not describe methods<\/strong> nor show images of self-harm.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">If there is imminent risk<\/h2>\n\n\n\n<p><strong>Do not leave alone<\/strong> to the teenager.<\/p>\n\n\n\n<p><strong>Remove media<\/strong> accessible dangerous.<\/p>\n\n\n\n<p>Contact with <strong>emergencies<\/strong> or the <strong>crisis resource<\/strong> local.<\/p>\n\n\n\n<p>Inform the <strong>family\/legal guardian<\/strong> and <strong>activate<\/strong> the clinical circuit.<\/p>\n\n\n\n<p><strong>Immediate help (Spain):<\/strong> Emergencies <strong>112<\/strong> \u00b7 24\/7 Suicide Hotline <strong>024<\/strong>.<br>If you are in another country, call the <strong>emergency services<\/strong> from your area or to a <strong>local helpline<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Frequent questions<\/h2>\n\n\n\n<p><strong>Does asking about suicide \u201cgive you ideas\u201d?<\/strong><br>No. Ask <strong>in a direct and respectful manner<\/strong> is associated with <strong>reduction<\/strong> of risk and <strong>greater access<\/strong> to help.<\/p>\n\n\n\n<p><strong>What if the self-harm is \u201cnot serious\u201d?<\/strong><br>Any self-harm <strong>deserves evaluation<\/strong>. The \u201cmild\u201d can escalate, and always indicates <strong>discomfort<\/strong>.<\/p>\n\n\n\n<p><strong>Can mediation be carried out during a crisis?<\/strong><br>The <strong>acute crisis<\/strong> is attended to first by <strong>mental health<\/strong>Mediation is useful <strong>after<\/strong>, to agree on support and routines.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Conclusion<\/h2>\n\n\n\n<p>It is not a \u201cfashion\u201d: it is <strong>real suffering<\/strong>. Detect <strong>soon<\/strong>, <strong>coordinate<\/strong> answers and <strong>accompany without judgment<\/strong> saves lives.
